We are seeking a compassionate, self-starter, and team-oriented professional to join our staff as a Community Care Worker, focusing on evening hours and working remotely.

About the Role

As a Community Care Worker with GroundGame Health, you will play a vital role in delivering population health services to assigned clients on an ongoing and emergency basis. This position requires a unique blend of technical skills, healthcare knowledge, and interpersonal abilities. You will utilize our proprietary platform/software and data management system to perform activities related to contracted services per intervention protocol, ensuring program goals are achieved. Your interventions may include meeting with clients at their residence, a location of their choice, or in hospital/facilities.

Key Responsibilities

  • Deliver Population Health Services: Provide services to assigned clients on an ongoing and emergency basis per contracted intervention protocol, within the contracted timeframe, by telephone or in clients' residences that may or may not be accessible by public transportation.
  • Develop and Maintain Program Knowledge: Develop and maintain an accurate and thorough knowledge of all program requirements, including specialized computer software and tools required to perform job tasks in a timely and accurate manner.
  • Ensure Compliance and Confidentiality: Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and GroundGame Health's confidentiality policies. Report any potential issues immediately.
  • Organize and Document Daily Schedule: Organize and document daily schedule to inform supervisor of activities, and adhere to safety protocols.
  • Meet Performance Expectations: Demonstrate competency by meeting or exceeding minimum caseload requirements and department work performance expectations.
  • Build Community Resource Knowledge: Develop and maintain knowledge of community resources available to assist clients; work with other departments to acquire knowledge of community resources available.
  • Communicate Effectively: Communicate with supervisor on a daily basis, and as needed, to report all areas of concern/issues/incidents to the State Account Manager.
  • Embody the GGH Standard of Excellence: Serve as an example of GGH Standard of Excellence by exemplifying GGH vision and goals, and demonstrate professionalism in relationships with customers/co-workers/supervisors.
  • Complete Administrative Tasks: Complete and attest to accuracy of payroll records and submit for review and processing according to GGH schedule.
  • Other Duties as Assigned: Perform other duties as assigned by supervisor.

Requirements and Qualifications

Education

We are looking for candidates with:

  • Associate's Degree in social work, or a related field; or
  • High school diploma, or general education degree (GED); and
  • Four years of related experience and/or training in healthcare and/or customer service position utilizing advanced technology tools, including customized software systems.

Experience

Preferred candidates will have:

  • Two years of experience and/or additional education in social work or a related field;
  • Knowledge of medical terminology and/or experience in a customer service position, preferably in a healthcare setting;
  • Certification as a Community Health Worker.

Additional Eligibility Requirements

Candidates must:

  • Possess and provide annually a valid driver's license;
  • Provide documentation of current automobile insurance at agency-specified levels;
  • Be available to work 5:00 pm - 8:00 pm and some weekends.
