Job Summary

We are seeking a licensed practical nurse with a passion for delivering exceptional patient care in a home health setting. As a Home Health LPN, you will work under the direction of a Registered Nurse (RN) to provide skilled nursing interventions, teaching, and training activities to patients with various illnesses, rehabilitative needs, and preventative care. Key Responsibilities:

  • Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, licensure/certification requirements, healthcare standards, and CenterWell Home Health policies and procedures.
  • Conduct patient assessments, develop and implement individualized plans of care, and evaluate patient outcomes.
  • Provide skilled nursing interventions, teaching, and training activities to patients and their families.
  • Utilize a holistic approach in the provision of problem-specific therapeutic interventions.
  • Confer with the RN Supervisor regarding needed changes in the Plan of Treatment.
  • Accept verbal orders from physicians, as permitted by state law/regulations/Nurse Practice Act, and communicate these orders to the RN Supervisor.
  • Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient/family needs and status changes.
  • Participate in care coordination activities and discharge planning as appropriate.
  • Maintain the highest standards of professional conduct in relation to confidential information.

Requirements:

Essential Qualifications:

  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

Preferred Qualifications:

  • Experience with electronic medical records (EMRs) and home health software.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
