About the Role

This is a full-time, field-based telecommuter opportunity in Oklahoma, offering a competitive salary of $26-$35 per hour. As a Care Management Coordinator, you'll be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the care management and care coordination process to facilitate appropriate healthcare outcomes for individuals.

Key Responsibilities

  • Utilize care management tools and data to conduct comprehensive assessments of referred members' needs/eligibility and recommend a course of action to address needs.
  • Identify high-risk factors and service needs that may impact member outcomes and care planning.
  • Coordinate and execute assigned care plan activities, monitoring progress and collaborating with interdisciplinary care teams.
  • Present cases at case conferences to obtain multidisciplinary reviews and achieve optimal outcomes.
  • Work cooperatively with members' healthcare providers, leveraging negotiation and influencing skills to secure necessary services and support.
  • Provide education, information, and support to empower members to make informed healthcare decisions.
  • Serve as a single point of contact for members, addressing immediate and acute gaps in care and access.
  • Monitor, evaluate, and document care using case management and quality management processes, adhering to regulatory and licensure rules and company policies.

Essential Qualifications

  • Reside in Oklahoma, within the specified regions.
  • 2+ years of related professional experience as a care manager, recovery-trained professional, health subject matter expert, or social service facilitator.
  • Completion of the Behavioral Health Caseworker certification training within 6 months of hire date.
  • 2+ years of experience using PC, keyboard navigation, and MS Office Suite applications.
  • Reliable transportation and willingness to travel in-state up to 30% of the time, with mileage reimbursement per company policy.
  • Ability to provide on-call coverage for CM inclusion on a rotational schedule as required by the State regulatory agency.
  • At least a Bachelor's degree in social work, psychology, or a related social service field.

Preferred Qualifications

  • Case management and discharge planning experience.
  • Managed Care experience.
  • Experience working with Native American/American Indian/Alaskan Native populations.
