Job Overview

We are seeking an experienced Inpatient Care Manager - BSN Remote Weekends to join our dynamic team. This hybrid role offers the flexibility to balance your time between our remote office and home, allowing you to work in a setting that suits your needs. As an Inpatient Care Manager, you will be responsible for coordinating patient care, assessing and evaluating patient needs, and developing individualized care plans to achieve optimal clinical and quality outcomes. You will work collaboratively with multidisciplinary healthcare teams to provide exceptional patient care and support the development and implementation of performance improvement activities.

Key Responsibilities

  • Assess patients at the point of entry to the healthcare system to identify expected length of stay, anticipated post-hospital care needs, and document all care in the EHR appropriately.
  • Develop, implement, and evaluate multidisciplinary, individualized patient plans of care that include transition plans.
  • Analyze patient outcomes and hospital performance indicators, making suggestions for areas of improvement.
  • Establish patient care management plans, interventions, treatment goals, and self-management goals, assessing the healthcare, educational, and psychosocial needs of patients and families.
  • Collaborate with multidisciplinary healthcare teams to provide patient and family education, support, and coordination of care.
  • Develop and maintain knowledge of Medicare, Medicaid, and key payer benefits and reimbursement methodologies.
  • Assist patients and families with self-management through education, visits, and telephonic engagement, encouraging and supporting patient adherence to care plans.
  • Communicate with physicians, patients, families, and other members of the healthcare team to coordinate transition planning.
  • Research, evaluate, and recommend resources to meet medical and non-medical needs of patients and families.

Essential Qualifications

  • Bachelor's Degree in Nursing
  • 3 years of experience in care coordination, disease management, home health care, hospital, or physician office setting
  • 1 year of prior electronic medical record (EMR) experience
  • Ability to incorporate strategies for interacting with persons from diverse backgrounds
  • Ability to set priorities to coordinate care plans efficiently
  • Knowledge of computer applications and analytical tools
  • Knowledge of nursing theory and application of learning theory
  • Proven leadership skills
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Preferred Qualifications

  • Master's Degree in Nursing
  • CCM - Certified Case Manager CCMC - State of Pennsylvania
  • ACM - Accredited Case Manager ACMA - State of Pennsylvania

Physical Demands

The physical demands of this role include lifting and carrying 25 lbs., frequent sitting and standing, frequent keyboard use, and patient care providers may be required to perform activities specific to their role, including kneeling, bending, squatting, and performing CPR.
