Your primary responsibilities will include: * Assessing the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration, and documentation. * Rendering and supervising care of clients on the unit, monitoring medication, and consulting with prescribers. * Providing appropriate health care and coordinating referrals as prescribed by providers. * Participating in the evaluation of client's condition and progress with team members. * Promoting education and adherence to prescribed treatment protocols. * Facilitating wellness and medically related psychoeducational groups to individuals. * Recording all client information within electronic medical record system daily. * Assisting RNs on a daily basis. * Offering nursing consult on emergency service evaluations. * Working with clients to coordinate care with their PCP, psychiatrist, and other professionals, including but not limited to medication refills and follow-up medical appointments. **Requirements** To be successful in this role, you will need to possess the following qualifications and skills: * Grade N04: Acute Care LPN * Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts. * Previous clinical experience working with substance abuse and mental health. * Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives. * All bilingual candidates are strongly encouraged to apply. * Current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work-related purposes, including transporting clients in your own vehicle. * All candidates will be subject to a CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services. **Preferred Qualifications** * Experience working in a detox or acute care setting. * Knowledge of medical, substance abuse, and mental health nursing. * Bilingual or multilingual skills. **What We Offer** As a valued member of our team, you can expect to receive: * A competitive salary and benefits package. * Opportunities for professional growth and development. * A dynamic and supportive work environment. * A comprehensive training program to ensure your success. * A sign-on bonus of $2,000 (eligibility will be confirmed during the interview process). * A chance to make a real difference in the lives of our patients and their families. **Our Culture** At UMass Memorial Health, we are committed to creating a culture of respect, inclusivity, and compassion.
