We're seeking an experienced LVN/LPN Care Management Professional to join our team as a Transition of Care Coach. This role offers a unique opportunity to balance work and life, with a hybrid schedule that combines remote work with flexibility to work from home. As a Transition of Care Coach, you'll play a critical role in ensuring seamless transitions for our members who are experiencing significant changes in their health status. You'll work closely with our care team to identify needs, facilitate referrals to clinical resources, and advocate for our members who cannot advocate for themselves.
About the Position
The Transition of Care Coach is a full-time telework position, requiring a strong and diverse skillset in relevant areas to drive success. You'll be responsible for completing outreach cadence calls and post-discharge questionnaires within required compliance-driven timelines, utilizing weekly and daily reporting to identify utilization for the purpose of reducing Emergency Department Utilization and 30-day hospital readmissions. You'll also facilitate Interdisciplinary Care Team Meetings with Social Services, Care Management, PCP, and other key players to discuss service needs and support safe transitions.
Responsibilities
- Complete outreach cadence calls and post-discharge questionnaires within required compliance-driven timelines.
- Utilize weekly and daily reporting to identify utilization for the purpose of reducing Emergency Department Utilization and 30-day hospital readmissions.
- Follow members identified as inpatient in hospitals (whether planned or unplanned admission) and throughout the subsequent care continuum until the member can return to prior level of functioning in the community.
- Facilitate Interdisciplinary Care Team Meetings with Social Services, Care Management, PCP, and other key players to discuss service needs and support safe transitions.
Requirements
To be successful in this role, you'll need:
- 2+ years of LPN or LVN nursing experience.
- Active and unrestricted compact LPN or LVN licensure in the state of residence.

- Bilingual (Spanish) skills, which are a plus!
- Self-motivation, energy, attention to detail, high organization, and technical savvy.
- Discharge planning experience.
- Advanced proficiency in Microsoft Word, Excel, and Outlook.
- The ability to multitask, prioritize, and effectively adapt to a fast-paced changing environment while providing outstanding care.
- Effective verbal and written communication skills.
