Job Summary

We are seeking a highly skilled and compassionate Hospice Medical Social Worker to join our team in Norfolk, NE. As a Hospice Medical Social Worker, you will play a vital role in our interdisciplinary team, providing essential support and care to patients and their families. This is a part-time, remote opportunity with a competitive salary range of $27-$38 per hour, depending on experience. Key Responsibilities

  • Assess and address the psychosocial, emotional, and financial needs of patients and their families, providing support and guidance to help them cope with their situation.
  • Develop and implement individualized plans of care, collaborating with the interdisciplinary team to ensure comprehensive care.
  • Provide education and support to patients, families, and caregivers on medical social work interventions and treatment plans.
  • Identify and address potential risks or threats to patient health, implementing interventions to mitigate these risks and promoting a safe environment.
  • Communicate effectively with patients, families, caregivers, and healthcare team members, ensuring that changes in patient status are promptly reported and addressed.
  • Perform care planning, transfer, and discharge functions, ensuring seamless transitions and continuity of care.
  • Participate in ongoing professional development, staying up-to-date on best practices and regulatory requirements.

Requirements and Qualifications

  • Master's or doctoral degree in Social Work from a school accredited by the Council on Social Work Education.
  • At least one year of experience in a healthcare setting, preferably in hospice or home health care.
  • Current licensure as a social worker in the state of Nebraska.
  • Current CPR certification.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, caregivers, and healthcare team members.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.

Preferred Qualifications

  • Experience with electronic medical records and other healthcare software.
  • Familiarity with hospice and home health care regulations and guidelines.
  • Certification in a related field, such as Certified Social Worker in Hospice and Palliative Care (CSWHP).
