Job Summary

The Social Work Lead for the Penn Partners in Care Program is responsible for the daily oversight of social workers who provide services to primary care practices. The successful candidate will have a strong background in social work, excellent leadership and communication skills, and the ability to work collaboratively with healthcare professionals to drive patient-centered care. Key Responsibilities

  • Provide professional guidance, oversight, and support to the PPiC Social Workers
  • Recommend and implement changes to develop the role of the PPiC Social Workers
  • Hire, onboard, and manage Social Workers, including maintaining an orientation and training program
  • Provide daily oversight of staffing, schedules, and payroll for the Social Workers
  • Manage referral pools and delegate daily assignments to the team
  • Prepare and deliver performance evaluations for the Social Workers
  • Maintain support for an individual panel of Social Work referrals in addition to supervisory responsibilities (up to 50% of total hours worked)
  • Collaborate with Clinical Leads and Program Director to develop and improve workflows and processes to support the Social Work role
  • Interact with hospital, home care, and post-acute program managers to promote effective communication channels to support patients
  • Interact with Practice Managers across the regions to assure coordinated and efficient delivery of care management services
  • Participate in committees or workgroups to support the improvement and development of the care management program
  • Run performance and volume reports for the team and review and analyze program data to assist in program evaluation and team productivity
  • Use quality improvement strategies to identify opportunities to improve services
  • Participate in payer initiatives that pertain to social services
  • Represent PPiC in community collaborative meetings for social work with other Philadelphia health systems
  • Coordinate with Penn-supported social resource volunteer groups to supplement PPiC social services to patients
  • Participate in PPiC program meetings
  • Represent PPiC with the social platform vendor (Findhelp) by attending relevant meetings
  • Provide staff support by participation in the PPiC staff meetings, SW meetings, and one-to-one staff meetings

Requirements and Qualifications

Essential Qualifications:

  • Master's Degree in Social Work required
  • Licensed Social Worker required
  • Dual PA & NJ License obtained within 6 months of hire required
  • 3+ years of relevant healthcare experience required
  • 1+ year of prior supervisory experience preferred

Preferred Qualifications:

  • Experience in care management, population health, or social work leadership
  • Knowledge of healthcare systems, patient-centered care, and social determinants of health
  • Excellent communication, leadership, and interpersonal skills
