Job Overview
As a Remote Licensed Therapist with our organization, you'll play a critical role in our integrated Care Team, working closely with a multidisciplinary group of healthcare professionals to deliver high-quality, patient-centered care. You'll utilize your expertise in assessing, diagnosing, and treating a range of behavioral health conditions, both chronic and acute, to develop personalized treatment plans that meet the unique needs of our members.
Key Responsibilities
- Assess and diagnose a range of behavioral health conditions, both chronic and acute, using evidence-based evaluation techniques.
 - Collaborate with a multidisciplinary health team to develop and implement personalized treatment plans tailored to individual member needs.
 - Conduct initial intakes and deliver therapeutic interventions through individual, family, and group sessions, utilizing trauma-informed and evidence-based psychotherapy approaches.
 - Facilitate access to community resources and services to support comprehensive behavioral health care.
 - Engage with primary care providers and specialists to monitor treatment progress, discuss care strategies, and ensure seamless coordination of care.
 - Provide psychoeducation to members, focusing on mental health promotion, preventive care, and wellness strategies.
 - Nurture trusting relationships with patients, aligning care with their goals, values, and cultural backgrounds.
 - Participate in multidisciplinary case conferences to ensure effective and holistic care, sharing expertise and insights to inform treatment decisions.
 - Document care and coordination efforts promptly and accurately in our electronic medical record (EMR) system, leveraging technology to enhance care coordination and outcomes.
 
Essential Qualifications and Skills
To succeed in this role, you'll need:
- 2+ years of outpatient experience working with adolescents and adults enrolled in Medicaid, Medicare, or dual-eligible programs.
 - 3+ years of experience in assessing, planning, and managing behavioral health care through direct clinical experience.
 - 1+ year of telemedicine experience, with a strong understanding of the unique opportunities and challenges presented by telehealth.
 - Active LCSW, LMFT, or LCPC license, with a strong commitment to staying current with best practices and emerging trends in behavioral health care.
 - Strong understanding of trauma-informed and evidence-based psychotherapy approaches, with a focus on delivering culturally sensitive care.
 - Proficiency in electronic medical record documentation, with a commitment to leveraging technology to enhance care coordination and outcomes.
 - Exceptional judgment and decision-making skills, with the ability to evaluate complex situations and develop effective treatment plans.
 - Effective collaboration and communication skills, with the ability to work effectively with diverse healthcare professionals and stakeholders.
 
Preferred Qualifications
While not required, the following qualifications are highly desirable:
- Candidates with MFT and CPC internships are encouraged to apply, bringing a strong foundation in behavioral health care and a commitment to ongoing learning and growth.
 - Experience working with diverse populations, including those with complex trauma, substance use disorders, or other specialized needs.
 - Knowledge of emerging trends and best practices in telehealth, including the use of technology to enhance care coordination and outcomes.
