About the Role
The Virtual Patient Manager is a key position responsible for managing relationships with skilled nursing facilities (SNFs) and home health agencies, ensuring seamless patient transitions, and driving initiatives to improve care performance and efficiency. This role is critical in achieving our goal of delivering high-quality, patient-centered care and improving outcomes for our patients.
Key Responsibilities
- Develop and maintain strong relationships with SNFs and Home Health Agencies to ensure effective collaboration and communication.
- Collaborate with internal stakeholders, including Clinical Strategists and Site of Care Manager, to communicate post-acute care interventions and drive care coordination.
- Engage with Skilled Nursing Facility Interdisciplinary Teams to achieve optimal length of stay for ACO and Value-Based Care patients and ensure patient stability post-discharge.
- Lead monthly meetings with leadership at each performance network facility/agency to report outcomes, identify areas for improvement, and recommend process enhancements.
- Analyze medical data and reports to identify high-priority care redesign interventions and drive performance improvement initiatives.
- Utilize CVS' patient tracking software platform daily to monitor patient care and outcomes.
- Confirm start of care dates for Home Health Agencies, including documentation of any delays or readmissions.
- Review and initiate SNF escalations for patients with social determinants of health (SDOH) or clinical barriers to discharge.
- Analyze readmission reasons and drive process improvements with SNFs and Home Health Agencies to reduce hospital readmissions.
Essential Qualifications
To succeed in this role, you'll need:
- A minimum of 6+ years of experience in healthcare and/or business, with a strong understanding of care transition models and post-acute care.
- A Bachelor's degree in a relevant field, such as healthcare administration, nursing, or business.
- Strong critical thinking and analytical skills to interpret medical data and reports.
- Patient assessment and evaluation experience, with the ability to exercise independent judgment.
- Excellent communication and interpersonal skills to build strong relationships with SNFs, Home Health Agencies, and internal stakeholders.
Preferred Qualifications
While not mandatory, the following qualifications are highly desirable:
- Case management and discharge planning experience.
- Understanding of Care Transition Models, such as Naylor, Coleman, and Boost.
- Experience working in a value-based care environment.
