Job Summary and Qualifications
As a PRN Physical Therapist Home Health, you will be responsible for providing high-quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients. This involves completing patient assessments, participating in developing and implementing individualized care plans, and providing or directing a majority of the patient's physical therapy. Your goal will be to support patients throughout the continuum of care, optimizing their physical abilities and skills.
Key Responsibilities
- Optimize patients' physical abilities and skills by performing or supervising therapy staff in initiating referrals, evaluating and treating appropriate patients, and effectively participating in the care planning process.
 - Establish personal patient schedules in conjunction with other team members and develop treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
 - Develop appropriate home programs to maintain and enhance the performance of the patient in their own environment and participate in discharge planning.
 - Provide, recommend, or fabricate adaptive devices, orthotics, or prosthetics, and train patients, families, and appropriate staff in their use or application.
 - Refer patients for treatment to Physical Therapy Assistants (PTA) by completing evaluations and initial plans of treatment and communicating information, then supervise and direct PTA in the implementation of the treatment plan.
 
Essential and Preferred Qualifications
To excel in this role, you should possess a Bachelor of Science in Physical Therapy from an accredited program and have completed the National Certification Examination for Registered Physical Therapist. A license to practice physical therapy within the state and the ability to work varying hours, including weekends and holidays, are also required. Being a licensed driver with an insured automobile in good working order is necessary for this position.
