Position Overview

Key Responsibilities

- Medication Administration: Administer medications and treatments as prescribed by healthcare providers, and monitor patients for any adverse reactions or concerns.
- Patient Monitoring: Continuously monitor patient health status and report any changes or concerns to the RN or physician, ensuring that patients receive timely and effective care.
- Care Planning: Assist with patient assessments and development of care plans, collaborating with the interdisciplinary team to ensure that each patient receives comprehensive care.
- Patient Education: Educate patients and their families about medical conditions, treatment plans, and self-care techniques, empowering them to take an active role in their health and wellness.
- Documentation: Accurately document patient care and maintain up-to-date medical records, ensuring that all patient information is current and easily accessible.
- Collaboration: Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care, communicating effectively with RNs, physicians, and other healthcare professionals.
- Regulatory Compliance: Adhere to all regulatory standards and organizational policies, maintaining the highest level of professionalism and integrity.
- Training and Supervision: Assist with training and supervision of nursing assistants and other staff members, providing guidance and support as needed.
Qualifications
- Licensure: Current licensure as a Licensed Practical Nurse (LPN) in the state of practice, with a strong understanding of nursing principles and practices.
- Experience: Previous experience in a healthcare setting preferred but not required; we're eager to welcome new LPNs to our team and provide the support and training they need to succeed.
