Overview of the Role

We're seeking an experienced and compassionate Home Health Nurse to join our team as a Hospice Case Manager in the Youngstown territory. This is a full-time, 1st shift position that offers a competitive salary, attractive benefits, and a $10,000 sign-on bonus. As a Hospice Case Manager, you'll play a critical role in delivering high-quality patient care, collaborating with our care team, and making a meaningful impact on the lives of patients and their families.

Key Responsibilities

• Performs comprehensive patient assessments and collaborates with the care team to develop and implement individualized plans of care.
• Makes referrals to other hospice care disciplines as needed and documents rationale for not doing so.
• Maintains accurate, timely, and detailed patient care progress records according to care center policies.
• Records pain/symptom management changes and outcomes as appropriate.
• Teaches patients, families, and caregivers essential patient care techniques.
• Regularly communicates with clinical managers and care teams to ensure seamless care coordination.
• Performs on-call responsibilities and provides on-call services to patients and families as assigned.
• Other duties as assigned.

Essential Qualifications

• One year of nursing experience, preferably in home health or hospice care.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Preferred Qualifications

• Experience working in a hospice or home health setting.
• Strong clinical skills and knowledge of hospice care principles.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Compensation, Perks, and Benefits

We offer a competitive salary range of $75,000 - $85,000, depending on location, job-related knowledge, skills, and experience. Our comprehensive benefits package includes:
