Experienced In-Home Geriatric Nurse Practitioner – Delivering Personalized Care to Elderly Patients in the Comfort of Their Own Homes

About the Role
As an In-Home Geriatric Nurse Practitioner, you will have the opportunity to work autonomously while maintaining ownership of your patient panel. Your daily responsibilities will include providing in-home primary care to our most frail and at-risk elderly patients, caring for an average of six to eight patients daily, and providing coverage for Metrowest communities. You will be an integral part of a cohesive team, working together to deliver high-quality patient care and making a significant difference in the lives of our patients.
Key Responsibilities:
- Provide in-home primary care to our most frail and at-risk elderly patients, developing personalized care plans to meet their unique needs and goals
- Have ownership of your patient panel, taking a proactive approach to managing their care and ensuring they receive the best possible outcomes
- Care for an average of six to eight patients daily, working efficiently and effectively to deliver high-quality care
- Provide coverage for Metrowest communities, traveling to patients' homes and working in a variety of settings
- Work autonomously as a member of a cohesive team, collaborating with other healthcare professionals to deliver comprehensive care
Essential Qualifications
To be successful in this role, you will need to possess a strong foundation in geriatric care, as well as excellent communication and interpersonal skills. You will be a graduate of an Accredited Nurse Practitioner Program, with a Master's level degree preferred. You will also be licensed by the Commonwealth of Massachusetts board of registration in Nursing and maintain BLS certification, professional certification as appropriate. Additionally, you will be fluent in EMR systems and technology, with a strong ability to navigate complex healthcare systems.
Preferred Qualifications:
- Minimum two years of APC Experience, with a focus on geriatric care
- Geriatric experience, with a deep understanding of the unique needs and challenges of elderly patients
- Homecare experience, with a proven ability to work effectively in a variety of settings
- National Board Certification, demonstrating your expertise and commitment to delivering high-quality care
