About the Role
We are seeking an experienced MSW Behavioral Health Case Manager to join our team in New Hyde Park, NY. As a Behavioral Health Case Manager, you will be responsible for providing case management and utilization review of behavioral health and substance abuse services. This will involve conducting patient assessments in home settings, determining appropriate levels of behavioral care, and managing cases throughout the entire treatment plan. You will work closely with our interdisciplinary care team to administer benefits, review treatment plans, and coordinate Medicaid/Medicare benefits and transitions between various areas of care.
Key Responsibilities
- Conduct patient assessments in home settings to determine appropriate levels of behavioral care needed
 - Obtain information from providers on outpatient requests for treatment and determine if additional clinical treatment sessions are needed
 - Manage behavioral health cases throughout the entire treatment plan, ensuring that patients receive comprehensive and coordinated care
 - Administer benefits and review treatment plans in collaboration with our interdisciplinary care team, including primary care providers and other healthcare professionals
 - Assist in coordinating Medicaid/Medicare benefits and transitions between various areas of care, ensuring that patients receive seamless and continuous care
 - Communicate with members who have complex medical needs and may have communication barriers, providing empathetic and patient-centered care
 - Identify ways to add value to treatment plans and consult with facility staff or outpatient care providers on those ideas, driving innovation and excellence in care
 - Focus on whole person care model for psychiatric and chemical dependency patients, addressing the physical, emotional, and social needs of our patients
 - Build relationships and work with a variety of populations within the community, including low-income populations with complex social and medical needs
 
Requirements and Qualifications
To be successful in this role, you will need to possess a Master's Degree in Social Work (MSW) and have at least 2 years of experience in long-term care, home health, hospice, public health, or assisted living. You should also have at least 1 year of clinical or case management experience and be proficient in MS Word, Excel, and Outlook. The ability to travel in designated territory within New York state is also required.
Preferred Qualifications
- Experience with electronic charting and arranging community resources
 - Experience working with complex family systems and dynamics, as well as crisis intervention
 - Experience working in Duals (DSNP) and/or Medicaid environment
 - Field-based work experience and knowledge of both Medical and Behavioral Health Diagnosis and terminology
 - Knowledge of symptom identification and intervention, associated with Behavioral Health and Substance Use disorders
 - Knowledge of APS reporting processes and resources to address SDOH
 - Background in managing populations with complex medical or behavioral needs
 - Understanding of health disparities among various membership, based on their race or ethnicity; religion; socioeconomic status; sexual orientation; gender, gender identity; age; mental health; cognitive, sensory or physical disability; geographic location or other characteristics historically linked to discrimination and exclusion
 - Proficient with Microsoft Word, Excel, Outlook, PowerPoint
 - Proven excellent written and verbal communication and relationship-building skills
 - Proven planning and organizational skills
 - Proven ability to work independently as well as a member of a team

Compensation and Benefits
We offer a competitive salary range of $58,300 to $114,300 annually, depending on experience and qualifications.
