Job Summary
We're seeking experienced nurses to work part-time from their home office as Chronic Care Management (CCM) and Remote Therapeutic Monitoring (RTM) Care Coordinators. As a Care Coordinator, you'll play a vital role in delivering high-quality, patient-centered care to individuals with chronic conditions. This is a unique opportunity to work independently, set your own schedule, and make a meaningful difference in the lives of our patients.
Key Responsibilities
- Conduct telephonic encounters with patients, develop detailed care plans, and perform care coordination activities
 - Collaborate with healthcare providers to ensure seamless care transitions and patient support
 - Utilize electronic health records to document patient interactions, care plans, and progress
 - Provide motivational coaching and education to patients, promoting healthy behaviors and disease management
 - Attend regularly scheduled meetings, including bi-monthly staff meetings and monthly one-on-one sessions
 - Meet productivity and quality standards, including completing encounters with 90% of assigned patients
 
Requirements and Qualifications
- Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
 - Current license to practice as an RN/LVN/LPN with no disciplinary actions noted
 - A minimum of two (2) years of clinical experience in Med/Surg, Case Management, and/or home health care
 - Proficiency in Electronic Medical Records and Windows desktop applications (Microsoft Office 365, Teams, Excel, etc.)
 - Strong organizational, time-management, problem-solving, and decision-making skills
 - Excellent verbal, written, and listening skills
 - High-speed internet and a desktop or laptop computer (Windows 10 or higher or Mac)

What We Offer
- Competitive pay structure: $10.00 per completed patient encounter up to 99 encounters/month, $10.25/encounter from 100-149 encounters/month, $12/encounter from 150-199 encounters/month, $14/encounter from 200-249 encounters/month, and $16/encounter for >250 encounters/month
