Position Purpose

As a Care Navigator, you will play a vital role in our organization, connecting potential clients to the correct Rosecrance services and resources. You will perform various duties, including customer service, client intake, and coordination with our team members. This position requires excellent communication skills, empathy, and a strong ability to navigate complex situations.

Key Responsibilities:

  • Customer Service and Client Intake: Answer incoming calls from the main line, greet callers, obtain and provide information through a set of predetermined questions, transfer calls to appropriate parties, and/or take messages.
  • Client Connection and Coordination: Open new client inquiries in our system, connect with the proper Access department or front desk, and ensure timely follow-up.
  • Fax and Document Management: Handle incoming faxes, scan and distribute documents to the proper staff or department, and maintain accurate records.
  • Team Collaboration: Work closely with the Access team to ensure calls for service are answered promptly and professionally, and participate in team meetings and activities as assigned.
  • Compliance and Code of Conduct: Uphold the Rosecrance Code of Conduct, adhere to our mission, vision, and values, and make decisions that align with our organizational framework.
  • Customer Service Excellence: Demonstrate strong customer service skills, incorporating the 5 tenets of the Rosecrance Way into daily practices and interactions with stakeholders.

Requirements and Qualifications

Essential Qualifications:

  • Education: High school diploma or GED, or an equivalent combination of education, training, and experience.
  • Experience: Minimum of one year of previous switchboard experience or an equivalent combination of education, training, and experience.

Preferred Qualifications:

  • Customer Service Experience: General customer service and switchboard experience are preferred.
  • Addiction Treatment Knowledge: Working knowledge of addiction treatment is beneficial.

To succeed as a Care Navigator, you should possess:

  • Excellent Communication Skills: Ability to effectively interact with callers, team members, and stakeholders.
  • Empathy and Compassion: Strong emotional intelligence and ability to provide empathetic support to clients and families.
  • Organizational and Time Management Skills: Ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records.
  • Technical Skills: Familiarity with software applications, including our client management system.
