Clinical Pharmacist - Remote Services
About the position
Responsibilities
• Establishes and maintains compliance with Brown University Health’s Flexible Work Arrangements Guidelines and Forms.
• Receives patient medication orders remotely when entered into the electronic health record (EHR) by a provider.
• Reviews and interprets each order to determine completeness and therapeutic appropriateness.
• Contacts the provider or covering provider as necessary to initiate appropriate interventions.
• Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications.
• Provides drug information and consultation to prescribers, nurses, and other health care professionals.
• Proactively reviews and approves medication orders in the EHR.
• Identifies, documents, resolves, monitors, and prevents drug therapy related problems.
• Engages with other clinical pharmacists for complex clinical situations.
• Integrates patient-focused medication therapy management and pharmaceutical care.
• Participates in pharmacokinetic dosing and monitoring of selected medications.
• Ensures compliance with target drug programs and medication management initiatives.
• Documents and reports all clinical interventions and medication errors.
• Remotely reviews and approves formulations for sterile products requiring compounding.
• Participates in continuous quality improvement activities.
• Maintains professional competence through participation in staff development programs.
• Performs other duties as assigned.
Requirements
• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
• Current unrestricted licensure as a licensed pharmacist or a licensed 90-day temporary pharmacist in the State of Rhode Island prior to start date.
• Current licensure as a registered pharmacist in the Commonwealth of Massachusetts prior to start date.
• Must possess and maintain Board of Pharmacy Specialties certification in area of clinical pharmacy expertise within 18 months of hire.
• Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or a minimum of 3 years clinical experience required.
• Effective interpersonal and communication skills, both written and verbal.
• Effective organizational and presentation skills.
• Knowledge of area/specialty specific technology including pharmacy information systems.
