About the Role: Bilingual Emergency Medical Technician (EMT)
We are seeking a highly skilled and compassionate Bilingual Emergency Medical Technician (EMT) to join our team as a remote Health Guide. As a bilingual EMT, you will be the critical link between patients, medical practices, and our Case Management team, ensuring seamless communication and care coordination. Your expertise will help patients navigate complex healthcare issues, address their concerns, and achieve better health outcomes.
Key Responsibilities:
- Collaborate with medical practice staff and Guidehealth's Case Management team to manage a targeted population of patients, ensuring comprehensive care coordination.
 - Function independently as an extension of the medical practice, utilizing your EMT expertise to address patient needs and concerns.
 - Engage patients through effective communication modalities (calls, texts, portals, emails) to promote better health outcomes and reduce medical spend.
 - Accurately document patient medical history and issues in the electronic health record (EHR), ensuring precise and timely data capture.
 - Understand and navigate the referral and prior authorization process, facilitating access to specialty care and community-based resources.
 - Build and maintain a trusting, professional relationship with assigned medical practices and within the Guidehealth organization.
 - Meet assigned Key Performance Indicators (KPIs) to drive positive patient health outcomes and reduce medical costs.
 - Escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or Guidehealth leadership as needed.
 - Occasionally travel to supported primary care physician offices to foster stronger relationships and improve care coordination.
 - Adhere to all security policies and procedures to protect patient data and Guidehealth intellectual properties.
 
Essential Qualifications:
- Bilingual (Korean/English) - Native or Fluent required.
 - Certified or Registered Medical Assistant, or certified EMT.
 - Proven experience in patient coordination, with a passion for serving individuals with complex medical and social needs.
 - Empathy and willingness to develop skills in motivational interviewing, critical listening, and behavioral change management.
 - Exceptional conversation skills, with the ability to precisely document patient interviews in real-time using software.
 - Knowledge of medical terminology, primary care physician practices, and social determinants of health.
 - Experience with electronic health records (EHRs), care management analytics databases, and various software programs.
 - Ability to balance competing priorities while remaining detail-oriented and focused.
 - Familiarity with a range of EHR systems, including EPIC, Athena, and Cerner.
 - Excellent written, visual, and verbal communication skills.
 - Ability to work independently as part of a patient's care team under the supervision of licensed clinical professionals.
 - Proven ability to thrive in a remote work environment.
 
Preferred Qualifications:
- Previous experience in care coordination, case management, or a related field.
 - Familiarity with healthcare technology, including telehealth platforms and patient engagement tools.
 - Knowledge of healthcare operations, including billing and coding practices.
