About Our Company and the Role
Molina Healthcare Services is a leading provider of Managed Long-Term Care (MLTC) and Medicaid Advantage (MAP) plans, streamlining the delivery of long-term services to those who need them most. As a Case Manager with our Senior Whole Health team, you'll play a critical role in assessing, planning, and coordinating care for our members, working closely with multidisciplinary teams to ensure seamless transitions and optimal outcomes.
This is a remote position that requires residency in New York. You'll work standard business hours (Monday - Friday, 8:30 AM - 5:00 PM) and be part of a dynamic team that's committed to delivering exceptional care and support to our members.
Key Responsibilities
- Conduct comprehensive clinical assessments to identify member needs and develop targeted case management plans.
 - Collaborate with members, caregivers, physicians, and other healthcare professionals to implement and monitor care plans.
 - Perform ongoing outreach and monitoring to evaluate progress, address concerns, and make adjustments as needed.
 - Promote integration of services, including behavioral health care and long-term supports, to enhance continuity of care.
 - Facilitate interdisciplinary care team meetings and informal collaborations to drive member-centered care.
 - Utilize motivational interviewing and clinical guideposts to educate, support, and motivate members.
 - Assess for barriers to care and provide coordination and assistance to address concerns.
 - Collaborate with RN case managers and supervisors as needed.
 - May be required to travel up to 40% locally, depending on member complexity and state-specific regulations.
 
Essential Qualifications
To succeed in this role, you'll need:
- A Bachelor's or Master's degree in a social science, psychology, gerontology, public health, or related field.
 - 1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
 - Active, unrestricted LCSW or LMSW licensure.
 - Valid driver's license with a good driving record and reliable transportation.
 - Strong organizational, time management, and communication skills.
 - Bilingual skills in Bengali or Russian are highly preferred.
 
Preferred Qualifications
While not required, the following qualifications can enhance your candidacy:
- 3-5 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
 - Managed Long-Term Care (MLTC) experience.
 - Case management experience with geriatric populations.
 - Ability to manage a high case load (up to 200 cases).
 - Certifications such as CCM, CHEP, LPC/LPCC, or LMFT.
