Geriatric Care Manager Lead – Social Worker (W2, Remote)

We’re hiring an experienced, compassionate, and operationally excellent Care Manager Lead. You’ll work full-time (W2) in a fully remote environment, managing your own caseload and serving as a dedicated partner to older adults and their families.
