As a Telephone Triage RN, you will play a vital role in supporting hospice patients and their families with compassionate care and guidance over the phone. You will provide thorough assessments through active listening and clinical questioning, document phone encounters and deliver clear instructions based on established protocols. Working collaboratively with patients, families, caregivers, and hospice staff, you will coordinate care needs, schedule nurse visits as appropriate, and arrange medications, supplies, and equipment to ensure seamless support. This role is essential in offering comfort, reassurance and timely interventions to those we serve. This is a full-time, remote position. A caregiver in this role will work 4 10-hour shifts. On-site training will be held at the Business Operations Center for 4-6 weeks before transitioning to remote work. You must live within 1 hour of Independence, OH and have at least 1 year of Hospice experience. A caregiver who excels in this role will: • Provide over the phone assessment through active listening and questioning. • Document phone encounters and provide hospice instruction based on protocols. • Coordinate patient care activities with patient, family, caregivers or hospice staff. • Effectively triage and provide resolution to all hospice department telephone calls • Accept verbal telephone tasks from physicians to add/change services • Assess both patient and physician needs and provide action or guidance to ensure needs are met • Conduct follow-up activities, including appropriate communication with patient/caregiver, agency staff, and physician and complete documentation of events and interventions. • Assist, review, research and resolve active patient and referral complaints and record outcomes accordingly to meet regulatory compliance standards. Minimum qualifications for the ideal future caregiver include: • Graduate of an accredited school of professional nursing • Current state licensure as a Registered Nurse (RN) • Minimum of one year nursing experience in an acute medical-surgical, critical care, home care or hospice setting • Strong customer service and computing skills Preferred qualifications for the ideal future caregiver include: • Bachelor’s of Science in Nursing (BSN) • Prior hospice experience • Prior telephone triage experience Our caregivers continue to create the best outcomes for our patients across each of our facilities. Must be able to hear normal conversation. • Requires periods of sitting and standing for extensive periods of time.
