Experienced Remote Licensed Healthcare Facilitator - Complex Care Management for Physical Needs Members

About the Role
As a Remote Licensed Healthcare Facilitator, you will be at the forefront of our care management activities, working closely with members, families, and healthcare providers to develop personalized care plans and education. This is a unique opportunity to make a real difference in the lives of our members, and we're looking for a professional with a strong background in healthcare facilitation to join our team.
Key Responsibilities
- Develop, assess, and facilitate complex care management activities for members with physical needs, focusing on high-quality, cost-effective healthcare outcomes
 - Evaluate member needs, barriers to care, and social determinants of health, recommending and facilitating plans for the best possible outcomes
 - Collaborate with providers to identify specialists, community resources, and services to address member unmet needs
 - Coordinate care between members, families, caregivers, and healthcare providers to ensure person-centered care and services
 - Provide ongoing follow-up and monitoring of member status, revising care plans as necessary to meet member goals and unmet needs
 - Facilitate care management and collaborate with providers to ensure timely access to needed care and services
 - Collect, document, and maintain member information and care management activities to ensure regulatory compliance
 - Provide education to members, families, and caregivers on disease processes, care options, and healthcare benefits
 - Offer feedback to leadership on opportunities to improve care and quality delivery
 - Perform other duties as assigned by leadership to meet business needs
 
Essential Qualifications
To be considered for this role, you must meet the following essential qualifications:
- A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
 - 2-4 years of related experience in healthcare facilitation or a similar field
 - RN - Registered Nurse - State Licensure and/or Compact State Licensure required
 - Residency and licensure in Florida
 
Preferred Qualifications
While not required, the following qualifications are strongly preferred:
- OBNICU (Observation, Burn, Neuro, ICU) experience
 - Behavioral health experience

Compensation and Benefits
Centene offers a competitive salary range of $54,000 - $97,100 per year, depending on your skills, experience, and education. Our comprehensive benefits package includes:
