About the Role

We are seeking an experienced and compassionate Hospice Case Manager - RN - PRN to join our team. As a member of our clinical team, you will lead by example and deliver professional nursing care to our patients in accordance with established plans of care. You will be responsible for completing comprehensive assessments, developing individualized plans of care, and providing skilled interventions to achieve realistic outcomes. Key Responsibilities

  • Deliver high-quality patient care through face-to-face interactions and accurate documentation
  • Conduct comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters
  • Develop and modify individualized plans of care that incorporate analysis of assessment data
  • Provide skilled interventions aimed at achieving realistic outcomes within a specified time period
  • Ensure compliance with federal and state regulations governing hospice care services
  • Perform clinical chart record reviews and provide orientation, training, or in-services to the care team as needed
  • Precept new staff and provide follow-up information to Agency Management
  • Determine if patients meet admission criteria and assist in the assignment of qualified personnel
  • Prepare clinical progress notes and review documentation on a continual basis to ensure compliance with hospice documentation standards

Essential Qualifications

To be successful in this role, you will need to possess the following essential qualifications:

  • Graduate of an accredited school of nursing, with a Bachelor's degree preferred
  • Current Registered Nurse licensure (in good standing) in the state(s) of practice
  • Current CPR certification
  • Valid driver's license and automobile insurance
  • One (1) year of experience as a nurse in hospice care or post-acute setting
  • Management experience is not required, but the ability to supervise hospice aides is necessary

Preferred Qualifications

While not essential, the following preferred qualifications will make you an even stronger candidate:

  • Experience with EHR systems and their implementation/optimization in complex clinical settings
  • Proficiency with federal, state, and governing agency regulations
  • Participation in continued clinical and in-service education, as well as professional training required by law
  • Knowledge and comfort level working with technology and enterprise-wide support systems and tools
  • Successful management of a high volume of work, changing priorities as needed

Compensation, Perks, and Benefits

We offer a competitive salary, with a range of $50,000 per year, depending on experience.
