Job Overview
As a Licensed Practical Nurse at Post-Acute Medical, you will play a vital role in our healthcare team, providing direct patient care under the supervision of a Registered Nurse. Your responsibilities will include administering medications, monitoring patient conditions, performing therapeutic procedures, and maintaining accurate and timely records. This position requires a strong foundation in nursing principles, excellent communication skills, and the ability to work effectively in a fast-paced environment. Key Responsibilities:
- Provide appropriate nursing care to assigned patients, as directed by an RN, ensuring that care is delivered in an age and population-appropriate manner.
- Administer oral and IV medications as prescribed, and chart appropriately and timely in the medical record.
- Review and update care plans as necessary, taking and recording vital signs, and providing ostomy care and wound dressing changes.
- Monitor patients for changes in medical condition, notifying the physician as appropriate, and collect specimens for laboratory analysis.
- Insert and care for nasogastric tubes, urinary catheters, and provide tracheostomy and ventilator care as needed.
- Collaborate in the development of the plan of care, including multidisciplinary planning, discharge planning, and patient/caregiver teaching for post-discharge care.
- Assess the need for community-based resources to support further care and observe patient progress, reporting changes in patient status to the RN and physician.
Essential Qualifications:
To be considered for this role, you must hold current licensure as a Licensed Practical Nurse/Licensed Vocational Nurse in the state where the hospital resides, along with current BLS certification. While one year of inpatient medical-surgical nursing experience is preferred, it is not required for consideration. You should possess verbal and written communication skills to effectively express ideas and views, and the ability to read and write English, including medical terminology and abbreviations. Additionally, you must have the cognitive ability to implement the nursing process with pediatric, adolescent, adult, and geriatric patients in the skilled nursing setting.
Preferred Qualifications:
- Prior experience in emergency or critical care environments.
- A higher comfort level working with vents, trachs, PICC lines, etc.
- Two years of experience in direct patient care.
