  • Supervise Field Activities: Oversee the performance of new hires and seasoned clinicians during home visits, providing real‑time feedback and corrective guidance.
  • Promote Regulatory Compliance: Educate clinicians on federal, state, and local healthcare laws, OASIS accuracy requirements, and CenterWell’s internal policies.
  • Serve as a Communication Hub: Act as the primary liaison between new hires, clinical leadership, peer mentors, and the Home Health Learning & Development team.
  • Track and Report KPIs: Monitor key performance indicators such as orientation completion rates, competency scores, retention metrics, and OASIS documentation accuracy.
  • Cultivate a Learning Culture: Continually assess learning needs, adapt educational content, and champion adult learning principles to ensure clinicians feel supported and motivated.
  • Travel to Field Sites: Spend up to 75% of time on the road, visiting a variety of patient homes and clinical facilities across the assigned region to provide on‑site mentorship.

Essential Qualifications – What You Must Bring

  • Education: Diploma or Associate’s Degree in Nursing (required). Bachelor’s or Master’s Degree in Nursing is highly preferred.
  • Licensure: Unrestricted RN license in the state where you will be practicing (Arizona for this position).
  • Experience: Minimum of 2‑3 years of direct home health experience, including familiarity with OASIS data collection and documentation.
  • Certifications: Current CPR certification and OASIS certification (or willingness to obtain within 90 days).
  • Leadership Background: Prior experience as a nurse leader, preceptor, or educator, demonstrating ability to guide and develop others.
  • Communication Skills: Strong oral and written communication abilities, with a proven track record of effective interpersonal interactions.
