Job Overview

The Licensed Practical Nurse (LPN) is a critical component of our home healthcare team, responsible for delivering quality patient care in accordance with developed care plans and physicians' orders. As an LPN with Aveanna Healthcare, you will have the opportunity to work autonomously in patients' homes, utilizing a range of skills to provide comprehensive care, including phone triage, patient education, observation and assessment, wound care, infusions, catheter care, and PICC line dressing changes. Key Responsibilities:

  • Deliver high-quality patient care in compliance with physician orders and under the direction of your Registered Nurse teammate
  • Perform hands-on client care, including documentation and timely communication with the client's physician and other caregivers
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk
  • Educate clients and their family members based on the client's specific needs
  • Provide care in patients' homes using a variety of skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes
  • Collaborate with our clinical team to develop and implement individualized care plans that meet the unique needs of each patient
  • Participate in ongoing education and training to enhance your skills and knowledge, staying up-to-date with the latest developments in home healthcare

Essential Qualifications:

To be considered for this exciting opportunity, you will need to meet the following essential qualifications:

  • An active LPN License in the state of Florida
  • Valid CPR certification
  • Basic understanding of Oasis and Medicare Skilled Nursing experience (preferred)
  • At least 1 year of LPN experience in a home health or healthcare setting (preferred)
  • Home Care Home Base experience (preferred)

Preferred Qualifications:

While not essential, the following qualifications are highly desirable:

  • Previous experience working in home healthcare, preferably with a Medicare-certified agency
  • Strong understanding of Oasis and Medicare regulations
  • Experience with electronic medical records (EMRs) and other healthcare technology
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
