Job Overview
We are seeking an experienced Field Health Social Services Coordinator to join our team in North Johnson County, TX. As a Field Health Social Services Coordinator, you will play a critical role in assessing, planning, and implementing care strategies that are individualized to each patient's needs. You will work closely with patients, families, and healthcare providers to ensure that patients receive the most appropriate and least restrictive level of care. This is a remote-based position that offers flexibility and autonomy, allowing you to work from the comfort of your own home while making a meaningful difference in the lives of others.
Primary Responsibilities
- Assess, plan, and implement care strategies that are individualized to each patient's needs and directed towards the most appropriate, least restrictive level of care
 - Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
 - Manage the care plan throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality, coordinated care and services are provided expeditiously to all members
 - Advocate for patients and families as needed to ensure that the patient's needs and choices are fully represented and supported by the healthcare team

Required Qualifications
To be successful in this role, you must possess:
- A current RN license in the state of Texas, or a Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Master Social Worker (LMSW) license
 - At least 2 years of experience working directly with people with Intellectual and Developmental Disabilities (IDD)
 - At least 2 years of experience working within the community health setting or in a healthcare role
 - Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
 - Reliable transportation and the ability to travel up to 75% of the time in the assigned region to visit Medicaid members in their homes and/or other settings
 - Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information (PHI)
 - Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service
 
Preferred Qualifications
While not required, the following qualifications are preferred:
- At least 1 year of experience with long-term care services and support, Medicaid, or Medicare
 - Ability to create, edit, save, and send documents, spreadsheets, and emails
 - Knowledge of the principles of most integrated settings, including federal and state requirements like the federal home and community-based settings regulations
 - Reside within a commutable distance of assigned duties
