Heart Transplant Operations - Transplant Coordinator II (TRANSPLANT CRD 2) - University of California - Sacramento, CA - work from home job Company: University of California Job description: The Transplant Coordinator (TC) coordinates care for adult and pediatric patients in end stage organ failure across the continuum of transplant care (pre-transplant, during transplant, and post-transplant), in addition to adults across the continuum of living donation (evaluation, inpatient, post-donation). The TC is the process expert regarding transplant phases of care and living donation. The TC educates patients, caregivers and hospital staff about transplant-specific topics and living donation. The TC collaborates with members of the health care team to enhance patient care and serves as investigator, consultant, and leader. The TC makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems. The Transplant Coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership to the organization. The program serves UCDH patients requiring solid organ transplantation.
