Job Summary

We're seeking an experienced Full Stack Licensed Practical Nurse (LPN) Clinical Coordinator and Care Manager to join our care management team in Marion County and surrounding areas, IN. As a Care Coordinator, you will be the primary care manager for a panel of older adult members with a variety of medical and/or behavioral health needs. You will be responsible for managing complex members, engaging with them face-to-face and/or telephonically, and developing and implementing individualized, person-centered care plans to support the best health and quality of life outcomes.

Key Responsibilities

As a Full Stack LPN Clinical Coordinator and Care Manager, your primary responsibilities will include:


  • Serving as primary care manager for high medical risks/needs members with comorbid behavioral health needs

  • Engaging members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic (SDoH) needs

  • Developing and implementing individualized, person-centered care plans inclusive of goals, opportunities, and interventions aligned with a person's readiness to change to support the best health and quality of life outcomes

  • Partnering and collaborating with internal care team, providers, and community resources/partners to implement care plan

  • Providing referral and linkage as appropriate and accepted by member (may include internal consult opportunities such as Housing Navigator, Pharmacy Team, Peer Specialist, etc. or community-based provider referrals such as PCP, specialists, medication-assisted therapy referrals, etc.)

Required Qualifications

To be successful in this role, you will need to have:


  • A Licensed Practical Nurse (LPN) with a minimum of 3+ years of clinical experience with older adults

  • 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH (e.g., food banks, non-emergent transportation, utility assistance, housing/rapid re-housing assistance, etc.)

  • 1+ years of experience with MS Office, including Word, Excel, and Outlook

  • 1+ years of experience in long-term support services or working with older adults

  • Resident of Indiana

  • Reliable transportation and the ability to travel within assigned territory to meet with members and providers

Preferred Qualifications

While not required, the following qualifications would be beneficial for this role:


  • 2+ years of experience in long-term support services or working with older adults

  • Experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care

  • Experience working in team-based care

  • Experience in serving individuals with co-occurring disorders (both mental health and substance use disorders)

  • Background in Managed Care

  • Bilingual in Spanish or other language specific to market populations

Physical Requirements

As a Full Stack LPN Clinical Coordinator and Care Manager, you will need to be able to:


  • Transition from office to field locations multiple times per day

  • Navigate multiple locations/terrains to visit employees, members, and/or providers

  • Remain stationary for long periods of time to complete computer or tablet work duties
