**Experienced Full Stack Perinatal Triage RN – Remote Maternal-Child Health Specialist**

Job Summary
We have an immediate opening for a dedicated Overnight/Weekend Perinatal Triage RN to join our remote team. As a key member of our team, you will provide and facilitate amazing patient-centered clinical care to our patients, working closely with our multidisciplinary teams to drive success. This role requires a strong and diverse skillset in relevant areas, including maternal-child health, telehealth, and patient engagement. Key Responsibilities
- Ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport
- Identifying and addressing barriers to care that have been identified by patients and/or the care team
- Reducing care gaps (missed appointments, medication management, etc.) by frequent and personalized engagements with patients
- Assessing the need for and educating patients on the equipment they will use to monitor their health remotely
- Timely response to abnormal diagnostic results (labs, radiology, etc.)
- Assessment of urgent concerns and proactively triaging patients to support appropriate utilization of emergency services
- Timely documentation of all care/interactions and escalating to appropriate multidisciplinary teams, as needed
- Supporting the development of programs and product by providing user feedback
Requirements
To be successful in this role, you will need to have:
- A current unrestricted multi-state (compact) license as a Registered Nurse
- At least 4 years of experience in a hospital and/or healthcare practice, serving maternal-child health populations (minimum 3 years in labor and delivery)
- Flexibility and agility in your thinking, with the ability to adapt to changing circumstances
- Reliable internet access and the ability to work remotely via video, phone, and text
- Occasional willingness to travel (infrequent)
- Ability to work nights, weekends, and holidays
- Understanding of the prevalence of birth inequity and the role that structural racism plays in maternal morbidity and mortality
- Experience providing care in Spanish (bonus points if you have this skill)
- Professional engagement and a commitment to delivering high-quality patient care
Preferred Qualifications
Bonus points if you have:
- Experience working with perinatal patients who have had complicated pregnancy-related conditions such as diabetes, hypertension, perinatal loss, etc.
- Experience providing virtual care

Compensation and Benefits
We offer a competitive salary range of $75,000-$80,000, based on paying competitively for our company's size and industry. Our total compensation package includes equity, benefits, and other opportunities at Pomelo Care.
