Certified Medical Assistant/REMOTE - Care Coordinator/Enrollment Specialist

In this hybrid role, you will carry out the following tasks

We are looking for a motivated, goals-driven and dynamic enrollment specialist to conduct outbound calls and enroll patients into Medicare's Chronic Care Management Program and Remote Patient Monitoring Program.

Duties include:

· Determine patient eligibility for CCM/RPM services.

· Contact patients by phone to enroll them into the program.

· Document in the electronic medical record and CCM/RPM software systems.

· Other duties as assigned.

Qualifications include:

Medical Assistant Certificate

· Ability to effectively promote Medicare's CCM/RPM program to older adults and persons with disabilities.

Sales experience and outbound call center experience preffered

· Communication skills, confidence, and assertiveness in promoting a product or service. Relevant sales experience required.

· Proficient computer skills. Able to accurately document in an electronic medical record and CCM/RPM software system.

· A team player who can follow workflows and processes to achieve goals.

· Able to maintain confidentiality and comply with all applicable laws regarding patient information.

· Self-motivated to meet enrollment goals.

    Additional Responsibilities
  • Reach out to patients to enroll them into Medicare's Remote Patient Monitoring/ Chronic care management program by telephone
  • Reach out to patients who are enrolled in Medicare's Remote Patient Monitoring/ Chronic care management program by telephone
  • Build relationships with patients and their family members, monitor patient physiologic data, and educate patients on ways to maintain wellness.
  • Provide over-the-phone coaching and support regarding the use of patient physiological monitoring devices, such as a blood pressure cuff or weight scale.
  • Use the clinic's electronic medical record to coordinate medical referrals and needs.
  • Complete health and social assessments to learn about their risks and how you can best help.
  • Support patients in their healthcare journey - and in their day-to-day lives!
    Requirements:
  • Current and valid CMA certification
  • 2+ years experience as a medical assistant
  • English
  • Fluency in Spanish (preferred not a direct requirement)
  • Technologically proficient
  • Have access to a quiet, distraction-free work environment
  • Maintain a reliable home internet connection of 60Mbps or more
