Job description: Posting Date 05/27/2025121 LINDEN AVE NE, ATLANTA, Georgia, 30308-2432, United States of AmericaWe are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team providing holistic care to kidney patients—specifically kidney transplant recipients in Atlanta, GA.This full-time hybrid-remote role includes both in-person and virtual responsibilities, combining the best of advanced practice nursing and nephrology care coordination.You will deliver high-quality care in a collaborative environment, performing Evaluation & Management (E&M) visits, reviewing labs, managing medications and referrals, and supporting complex patients through the healthcare system. Your efforts will directly support transplant recipients in their journey to support their new kidney and live a healthy life.Key Responsibilities:Perform in-person Evaluation & Management (E&M) appointments for transplant recipientsReview labs and clinical data to make timely care plan adjustments in collaboration with supervising nephrologistsManage and adjust immunosuppressant therapies and chronic disease medicationsHandle prescription management, refills, and patient referralsMaintain a thorough and comprehensive EHR for each patient, including accurately recording the appropriate diagnosis codes and treatment codesSchedule & Work Environment:Full-Time, Monday–Friday. Hybrid Remote.
