Remote Hospice Triage LPN PT Weekend (Sat & Sun) only
About the position
IntellaTriage is seeking dedicated Hospice Triage Nurses to join our growing team. Our nurses enjoy the flexibility of working from home, providing essential care without the stress and expense of daily commuting. This position is designed for those who are passionate about improving the lives of patients and their families, particularly in hospice and end-of-life care settings. Since our inception in 2008, we have been committed to delivering compassionate, patient-centered care through our after-hours nurse triage services for hospice and home health providers. Our team of licensed, registered nurses utilizes best-in-class technology to ensure that patients receive the care they need, when they need it, based on client-customized protocols. As a Hospice Triage Nurse, you will become an integral part of our clients' care teams, providing support during non-core hours. You will be responsible for managing after-hours care delivery, ensuring that patients receive timely and appropriate responses to their needs. This role requires a strong background in hospice, palliative, and end-of-life care, as well as experience in fast-paced environments such as emergency departments, critical care, or surgical services. You will receive three weeks of remote paid training to prepare you for this role, with the training schedule varying based on your availability. In this position, you will have the opportunity to pick up additional shifts as they become available, allowing for greater flexibility in your work schedule. We provide our nurses with the necessary equipment, including a laptop and headset, while you will need to ensure you have a reliable high-speed internet connection and a quiet, HIPAA-compliant workspace. You will be expected to access electronic medical records (EMRs) for charting and utilize our internal applications to perform your job functions effectively. This role requires strong technological proficiency, the ability to follow instructions, and excellent communication skills to interact empathetically with patients and their families.
- Responsibilities
- Provide after-hours triage support for hospice and home health clients. ,
- Utilize client-customized protocols to deliver compassionate care to patients. ,
- Access EMRs for charting and utilize internal applications for job functions. ,
- Communicate empathically with patients while adhering to established protocols. ,
- Handle multiple calls efficiently, managing a minimum of 5 calls per hour on weekdays and up to 8 on weekends. ,
- Complete required charting and documentation at the end of each shift. ,
- Participate in ongoing training and maintain continuing education units (CEUs) as required.
- Requirements
- Must have or be willing to obtain a Compact LPN license. ,
- Experience in hospice, palliative, and end-of-life care is required. ,
- Experience in a fast-paced environment such as emergency departments, surgical services, or critical care is preferred. ,
- Must be comfortable accessing multiple technology applications for documentation during calls. ,
- Ability to type quickly and accurately in a fast-paced environment is essential. ,
- Fluency in English is required; additional languages are a bonus. ,
- Must reside in the U.S. and be legally eligible to work for any employer. ,
- Must complete three weeks of remote paid training during days and evenings. ,
- Must be available to work holidays as required. ,
- Must be able to handle stress and multitask effectively.
