We are seeking a skilled and compassionate Nurse to work from the comfort of their own home, performing telephonic encounters with patients, developing detailed care plans, and providing exceptional support to our partners. As an Independent 1099 Contractor, you will have the autonomy to set your own hours, with the potential to transition to a full-time W2 employment position with benefits.
Key Responsibilities:
- Conduct telephonic encounters with patients, developing and implementing personalized care plans to prevent and manage chronic conditions
 - Collaborate with healthcare providers to ensure seamless care coordination and communication
 - Utilize our electronic health record system to document patient interactions, care plans, and outcomes
 - Work independently, prioritizing tasks and managing time effectively to meet productivity goals
 - Participate in regular meetings and training sessions to stay up-to-date on best practices and industry developments

Requirements:
- Graduate of an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
 - Current license to practice as an RN/LVN/LPN, with no disciplinary actions noted
 - Minimum 2 years of clinical experience in Med/Surg, Case Management, and/or home health care
 - Proficiency in Electronic Medical Records, Microsoft Office 365, and other computer applications
 - Excellent verbal, written, and listening skills, with strong phone etiquette and customer service skills
