We are seeking a dedicated and skilled Field RN Care Coordinator to join our team in Delta, Franklin, Titus, Wood Counties, TX. As a Field RN Care Coordinator, you will play a vital role in our Integrated Care Model, advocating for the best possible care for our members and ensuring they receive the right services to meet their needs. This is a remote position with a home-based office, offering a competitive salary and a comprehensive benefits package.

We work closely with nursing facilities to provide coordinated care and support to our members.
Primary Responsibilities:
- Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care
- Identify and initiate referrals for member transition to community request and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders for the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the healthcare team
- Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices

Required Qualifications:
- Current and unrestricted RN license in the state of Texas
- 2+ years of experience working within the community health setting or in a healthcare role
- Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications
- Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
Preferred Qualifications:
- 1+ years of experience with long-term care services and support, Medicaid or Medicare
- Ability to create, edit, save, and send documents, spreadsheets, and emails
- Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
