We're seeking a compassionate and skilled Care Coordinator to join our team at Boston Health Care for the Homeless Program (BHCHP). As a Care Coordinator, you will play a vital role in connecting individuals experiencing homelessness with serious mental illness, substance use disorders, and complex medical illnesses to essential care and services. This hybrid role offers a unique blend of in-person and remote work, with 3 days on-site and 2 days remote.
At BHCHP, we're dedicated to providing unconditional, equitable, and dignified access to high-quality healthcare for all individuals and families experiencing homelessness in the Greater Boston area. Our team of clinicians, case managers, and behavioral health professionals work tirelessly to serve over 10,000 homeless individuals each year, providing comprehensive healthcare services that address social determinants of health and promote dignity, compassion, and mutual respect.
About the Role:
- Conducting primary outreach attempts to new assignments, including telephonic and in-person outreach to streets, shelters, medical facilities, and other locations
 - Completing intake paperwork, comprehensive assessments, and care plans with enrollees
 - Using multiple information technology platforms to locate and engage patients
 - Triaging and troubleshooting care need issues for enrollees and working with other team members to address needs and engage patients
 - Participating in BH CP training and learning collaboratives
 - Requesting and sending medical records to providers, ACO, MCO as needed

Requirements:
- Excellent customer service skills and the ability to communicate professionally with employees and enrollees
 - Efficient, organized, and detail-oriented, with the ability to complete tasks in a time-sensitive manner
 - Experience working with homeless populations and/or populations with mental illness and substance use disorders (preferred but not required)
 - Strong computer skills, including proficiency with Microsoft Excel Spreadsheets and database entry
 - Bilingual English/Spanish preferred
