As a Care Manager, you will play a vital role in connecting with and supporting individuals with complex health needs. You will build relationships, assess needs, develop care plans, and provide guidance to help individuals navigate the healthcare system and achieve their goals. Key Responsibilities:
- Conduct comprehensive assessments of assigned individuals' needs, qualifications, and goals using care management tools and information gathering techniques.
- Develop and implement individualized care plans, coordinating with healthcare providers, social service agencies, and other relevant stakeholders.
- Provide education, support, and advocacy to individuals, empowering them to make informed healthcare decisions.
- Monitor care plan progress, ensuring individuals are receiving appropriate services and achieving desired outcomes.
- Identify and address barriers to care, collaborating with stakeholders to overcome obstacles.
- Maintain accurate and up-to-date documentation in accordance with regulatory requirements.
- Participate in case conferences and multidisciplinary team meetings.
Qualifications:
- Minimum of 2 years of experience in behavioral health or social services.
- Ability to travel up to 50-75% of the time to meet with individuals in person within Champaign/Vermilion Counties, IL and surrounding areas.
- Reliable transportation is required; mileage is reimbursed according to company policy.
- Valid IL BH clinical permit – LCSW, LCPC, LMFT required.
Preferred Qualifications:
- Experience in case management and discharge planning.
- Experience with oversight care.
- Proficiency in Microsoft Office Suite.
Education:
Bachelor's degree required, with a degree in behavioral health, social work, or a related field.
