Care Coordinator - Bilingual (English/Spanish)
Description:
• Assessing transitional care needs
• Coordinating care across the continuum
• Engaging with patient and family to assure care needs are met
• Implementing timely, safe patient discharge plans
• Providing psychosocial assessments
• Offering guidance to patients/families on community-based services.
• Advocating for patient and family involvement in care decisions.
• Documenting assessments and interactions in medical records.
Requirements:
• Bachelors Social Work or Masters Social Work-Preferred
• BLS - Basic Life Support or BLS-I - Basic Life Support - Instructor
• Minimum 1 year of experience in healthcare in the acute care setting, related field or skilled care environment or community or educational internship in care coordination.
• Excellent written and verbal communication skill.
• Strong assessment, interview, organizational and problem-solving skills.
