Summary

Under general direction of the Chief of Geriatrics, assumes full responsibility for the rendering of professional medical services to a panel of older adult patients in their home setting including patients’ homes and Assisted and Independent living facilities. Serves as the primary care provider for geriatric patients unable to travel to an ambulatory practice location. Works in a collaborative team practice that recognizes the patient as the center of the team.Promotes enhanced continuity, visible teamwork, and the establishment of trusting and effective individual relationships with patients.Provides the full scope of primary care services within their licensure, education, and training. Practices in accordance with contemporary, evidence-based practice. Maintains a strong working relationship with the Primary Care team, Hospice, Home Health, Palliative Care, Specialties and Case Management. Provides leadership presence to the primary care team including but not limited to other PCPs, APCs, RNs, MAs, pharmacists, case managers, and population health managers. Acts as a mentor and role model to professional staff within the department.

EDUCATION/LICENSES/CERTIFICATIONS

Graduate of a State-approved school of Nursing and Nurse Practitioner program. Master of Science degree and Nurse Practitioner certification in an area of specialty required. Possess a current license to practice as a professional nurse in the Commonwealth of Massachusetts, and authorization to practice in the expanded role. Possession and maintenance of or eligibility for DEA and Mass DPH Controlled Substances Registration. American Heart Association Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) may be required based on specialty.

Experience

A minimum of 2 years of clinical experience in a Primary Care, Geriatrics, ED, or similar setting is required. Prior geriatric/older adult patience care experience strongly preferred. Experience with an electronic medical records system; EPIC experience preferred. Experience visiting patients in their homes preferred.

Skills

Must exhibit strong critical thinking, problem solving, interpersonal and good patient interviewing skills. Highly motivated and self-directed. Ability to interpret clinical information, assess the implication of treatment and develop and implement a plan of care. Previous experience with patient panel management preferred. Demonstrated ability to prioritize, multitask, and work in a rapidly changing environment with multiple demands. Demonstrated excellence in practice, documentation, and utilization. Maintain high patient satisfaction. Ability to work effectively as part of a team. Strong computer skills
