We're seeking an experienced Field Behavioral Health Care Coordinator/SPMI to join our team in North Harris County, TX. As a key member of our Integrated Care Model, you'll work in the field to coordinate the long-term care needs for patients in the local community. With the support of an elite and dynamic team, you'll use your diverse knowledge and experience to make health care work better for our patients.Key Responsibilities
- Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team.
- Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

Requirements
To be successful in this role, you'll need:- One of the following licenses:
- RN in the state of Texas
- Licensed Professional Counselor (LPC)
- Licensed Bachelor Social Worker (LBSW)
- Licensed Masters Social Worker (LMSW)
- 2+ years of experience working within the behavioral health setting
- Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
- Reliable transportation and the ability to travel up to 75% of the time in this assigned region to visit Medicaid members in their homes and/or other settings
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications
While not required, we're looking for candidates with:- Medicaid Waiver experience
- Case Management and/or Care Coordination experience
- Behavioral health experience
- Field-based work experience
- Experience with electronic charting
- Background in managing populations with complex medical or behavioral needs
- Knowledge of community resources
