Key Responsibilities and Expectations

As a Community Health Nurse at Amedisys, you will play a vital role in delivering high-quality patient care and promoting patient health and independence. Your key responsibilities will include:

  • Performing patient assessments and collaborating with the care team to develop and implement a plan of care
  • Making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so
  • Promoting patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques
  • Supervising LPNs and HHAs to ensure that patients receive comprehensive and coordinated care
  • Completing documentation timely and accurately to ensure that patient records are up-to-date and reflective of their care
  • Regularly communicating patient progress to the clinical manager and care team to ensure that everyone is informed and involved in the patient's care
  • Planning and providing staff education to promote ongoing learning and professional development
  • Performing on-call responsibilities and on-call services to patients/families as assigned to ensure that patients receive timely and effective care
  • Participating in clinical development and continuing education programs to stay current with best practices and industry developments
  • Other duties as assigned to support the overall goals and objectives of the team

Essential and Preferred Qualifications

To be successful in this role, you will need to possess the following essential qualifications:

  • One (1+) year of clinical experience as a Registered Nurse (RN), with a strong preference for experience in home health care
  • Current RN license, specific to the state(s) you are assigned to work, with a strong understanding of state and federal regulations governing home health care
  • Current CPR certification, with a commitment to maintaining certification and staying current with industry developments
  • Valid driver's license, reliable transportation, and liability insurance, with a willingness to travel to patient homes and other locations as needed

Preferred qualifications include:

  • Experience working with electronic medical records (EMRs) and other healthcare technology systems
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex patient data and develop effective care plans

Compensation, Perks, and Benefits

As a Community Health Nurse at Amedisys, you will be eligible for a competitive compensation package, including:

  • A salary range of $70,200.00 to $78,000.00, depending on experience and qualifications
